A simple application will be filled out on your first visit, which should take approximately 10 minutes. All other paperwork will be given to you to be filled out at home if your case goes on to step two.
All paperwork will be FedEx mailed out directly to you after an appointment has been made. A waiting list exists of approximately 2-4 weeks.
